How to Fill Out the Japanese Booking Site (Instant Reservation)
This page explains how to make an immediate reservation using the Japanese booking site.
Although the form is in Japanese, you can complete it in English by following the examples shown below.
If you prefer to see the questions in English, please use the 【English Booking Form】.For appointments after midnight (12:00 AM), please use this form.
For general inquiries or changes to your reservation, contact us via 【Contact Us】.
This booking form is not for instant reservations (we’ll reply within 48 hours).
However, you can write in English, so feel free to use it if that’s easier for you.
Please provide three possible appointment dates and times.We will review your availability and send you a confirmation email with the finalized appointment.

Instant Reservation via Japanese Booking Site (EPARK歯科)
To confirm your reservation immediately, please use our official booking site (EPARK歯科), which is in Japanese. You may fill out the form in English.

Entering Japanese Hiragana
There is a section where you’ll need to enter “なまえ” in Japanese hiragana.
If you can’t type in hiragana, please open this page on your smartphone and copy & paste the example text.
Free Account Required on “EPARK”
Creating a free account on the EPARK Dental site is required to make a reservation. Don’t worry — it’s free of charge.
Reservation Details via Email
Reservation details will be sent to the email address you provide in the form. These emails will be in Japanese.
Questions or Changes — Contact Us in English
If you have trouble understanding the instructions or wish to make changes or cancel your appointment, please use our inquiry form. You can enter your name & email address and contact us there — communication in English is possible through that channel.
